If file was removed and sorted in the completion model, sometimes the
view is trying to access the removed data, so check if requested item
is in the model.

Previously file monitor and folder monitor was operating on the same
event in 2 threads. Now the logic is moved into the folder monitor.

Also folder-monitor handler is improved.
