"Focus follows mouse" acts inconsistiently

Issue Description

The "Focus follows mouse" mode occasionally does not properly focus the window underneath it and may require further clicking and interaction with the window elements or decorations before focus is handed off properly.

Steps to Reproduce Issue

I'm still not sure exactly what triggers this exactly, so apologies in advance if this seems uselessly vague.

  1. Interact with windows for a while with "Focus follows mouse" on.
  2. At some point, a window will not focus fully when the mouse moves over it. Key presses may not be passed into the application until the window contents or decorations are interacted with using the mouse buttons.

Expected Behavior

Focus is always fully passed to windows that have their mouse cursor above them. Key presses always pass into the application that should be focused under these circumstances.
