Dialog to edit keyboard settings - Broken on Add

bring up dialog of keyboard shortcuts
click the ADD button
entery any command, ==> note no way to select a keyboard shortcut ??
click OK,
note the SHORTCUT field is blank missing and no OK button only close! in next dialog.
